Hi John, 
On the 1930, 1940 Census and WWII Draft Registration, Joseph Sr. had Yugoslavia as his Birth Country, so I don't know. 
As far as the city, I think Joe said there was still Ponikvar's on his dad's side still alive, he may be able to find out.  I did find 
Joseph Sr. and Frances Prelesnik's  marriage recorded in Leadville, Lake County, Colorado.  They were married 14-May-1911. 
Its on Film #001690121 in Family Search.  I was surprised to find they were married in Leadville.  There are other Ponikvars
listed in the 1930 Census for Leadville, although I don't know if they are related...the Parents show Yugoslavia as their birth, 
several kids all listed Colorado.

I think the headstone is the correct date.

John has two headstones, one says Johnnie Ponikvar, the other is a Military headstone, it says

Frances Ponikvar (daughter) 
From the Census she would have been born in 1908-1909.  She was 21 in 1930.  She had a daughter
named Mary that is listed as a Grand Daughter, 7 months.  I think Frances had two kids, they usually referred to 
them with a nick name.   Joey would be able to give you that information.  I didn't know Frances and the kids 
very well. 

Julie Gizewski Ponikvar ( Joe's wife, Joey's mother) 
Birth:  3-May-1915 
Death:  10-June-1951

The Historical Research Center Family Name History - Ponikvar

The Slovenian family name Ponikvar is classified as being of habitation origin. The phrase “habitation names” is used to describe those family names which find their origins in the location of the residence of the initial bearer. In some instance such names are derived from the name of the home or region where the original bearer was born or resided. Others refer to the geographic location of the original bearer’s home while still others may refer to a sign displayed over the door of his residence. With regard to the family name Ponikvar, the appellation may be traced to the Slovenian town of Ponikvari (today know as “Ponikva”), which lies east of Celje on the river Glina, a tributary of a branch of the Save. Prior to 1918, Ponikvari was governed as part of the Austro-Hungarian province of Croatia-Slavonia and it was within the boundaries of the area historically known as the “Banat”, a name derived from an old Persian term denoting a military district. Thus, the original bearer of the family name Ponikvar would have been one who dwelled in Ponikvari, or a native of that town who had migrated to another, perhaps nearby, settlement.

The present inhabitants of Slovenia first settled in that region at the end of the eighth century, having migrated there from neighboring Dalmatia, and were converted to Christianity by Saint Cyril and Methodius in the following century. Research indicates that the use of hereditary family names in Slovenia began as early as the thirteenth century and that the practice has been widely accepted by the sixteenth century. To date, there is no record of a coat of arms for this surname.
